Patient Portal

Our clinic offers a Patient Portal application for patients to:

• Access and view lab results
• View and generate electronic version of urgent care record
• Secure communication between you and your provider

Creating a Patient Portal account is easy:

1. Visit login.intelichart.com or visit the Intelichart app by downloading it from the Apple App Store or Google Play
2. Create an account by entering your PIN (please request this from the FO) and your date of birth
3. Complete the registration process
4. Once registration is completed, you will receive a verification link via email
