Our clinic offers a Patient Portal application for patients to:
- Access and view lab results
- View and generate electronic version of urgent care record
- Secure communication between you and your provider
Creating a Patient Portal account is easy:
- Visit login.intelichart.com or visit the Intelichart app by downloading it from the Apple App Store or Google Play
- Create an account by entering your PIN (please request this from the FO) and your date of birth
- Complete the registration process
- Once registration is completed, you will receive a verification link via email
